Description

This expanded recall involves Philips Lighting Energy Advantage Ceramic Metal Halide Lamps model CDM330 manufactured from May 2011 through March 2014. They are designed as energy efficient replacements for traditional 400W quartz metal halide lamps installed in magnetic ballasts and intended for use in high-ceiling industrial, retail and commercial applications. The lamps were sold in both clear and coated versions. Each lamp includes an etching, located either on the base of the lamp or the glass bulb that displays the date code in the format of a number followed by a letter representing the year and month, “Philips Lighting”, wattage (330W) and the model number (CDM330). Lamps with the following date codes are included in the recall:

Each lamp includes an etching, located either at the base of the lamp or on the ovoid of the lamps, that displays the relevant date code, along with Philips Lighting's name, wattage (330W) and the model (CDM330).

Consumers should immediately stop using the recalled lamps and contact Philips Lighting for a free replacement.

What problem has Philips Lighting identified with the “Energy Advantage” 330W Ceramic Metal Halide lamps with AllStart Technology (CDM330)?

Philips Lighting recently discovered that, during normal end-of-life failure, the outer glass bulb may shatter and can result in heated internal lamp particles and pieces of glass falling from the ceiling. Philips Lighting is committed to the well-being of its consumers and has decided to voluntarily recall the affected products and replace them at no cost to the end-user.
What are you doing to resolve the problem? 

Philips Lighting is offering to exchange affected lamps and provide end-users with a free replacement lamp.

Is the exchange program free of charge?

Yes. If you own one or more affected products, you will receive free replacement product upon contacting Philips Lighting.

What is Philips Lighting’s advice to owners of a Metal Halide lamp?

Owners of affected units should immediately discontinue using the product and call (800) 634-0138 (M-F 9:00am – 5:00pm ET) or send an email to [email protected] to register for a free replacement. You may also visit philips.com and click “Recalls” under the “For Professionals” tab.
Can I continue to use my Philips Lighting Metal Halide lamp?

No, you should immediately discontinue use of affected lamps and contact Philips Lighting.

How should I dispose of affected lamps?

Because these lamps contain mercury, there are strict requirements that govern their disposal. You must dispose of these lamps in accordance with all federal, state and local laws and regulations.

Affected lamps are marked with the one of the relevant date codes along with “CDM330” as the model, as shown below. Affected units will also be marked with “USA” as the country of origin.
Does the problem occur with all Philips Lighting Ceramic Metal Halide 330W lamps?

No, the risk is limited to a small batch of lamps with the specified date codes as indicated above. All other lamps are unaffected.

Where can I find the UPC Code?

The UPC number can be found on the Bar Code on the lamp package.  The Bar Code, without the first digit, contains the UPC code that coincides with the list of products that is included in the recall.

How long will it take to replace the affected product?

Once you have registered your affected lamp and Philips Lighting has received and confirmed it was from an affected batch, you will be sent a replacement lamp. You can expect to receive your replacement lamps approximately two to four weeks upon contacting Philips Lighting: via phone @ (800) 634-0138 or via email @ [email protected].
