The outer glass lens may shatter as a result of being exposed to the normal heat and pressure inside of these light bulbs, causing in pieces of glass falling from the ceiling. Additionally, the glue which attaches the glass lens to the body of the bulb can fail and cause the lens to fall and potentially shatter upon impact. Note also that the interior of the light bulb is hot, and thus, until the glass cools, the falling glass also presents a risk of burning. Accordingly, we have initiated a voluntary recall of the affected units in cooperation with the Consumer Product Safety Commission (“CPSC”).
This recall only involves Philips Halogen 60W PAR16 120V lamps manufactured between November 2013 through March 2015 (date codes: 3L, 3M, 4A, 4B, 4C, 4D, 4E, 4F, 4G, 4H, 4J, 4K 4L, 4M, 5A, 5B, 5C). All affected lamps are marked with “China” as their country of origin and “60W/120V” as the model.
Philips is committed to the well-being of its consumers and has decided to voluntarily recall the affected Halogen 60W PAR16 lamps and replace them.
Philips is offering to exchange affected lamps and provide consumers with a free replacement lamp.

Our advice to owners is to check whether their Philips Halogen PAR16 lamp is within the scope of this voluntary recall. No, the risk is limited to a batch of Halogen lamps produced at one manufacturing location with the above specific model number and date codes as indicated above. All other Halogen PAR16 lamps are not at risk and completely safe to use.

Once you have registered your affected lamp and Philips has received and confirmed it was from an affected batch, you will be sent a replacement lamp. You can expect to receive your replacement LED lamp approximately four weeks after you make contact with Philips Customer Care either via phone @ (800) 555-0050, via E-mail @ email@example.com or by visiting the company’s website at www.philips.com/recall. We appreciate your cooperation and sincerely regret any inconvenience caused.
The CPSC is aware of the problem and we are working jointly with them to provide information to Philips Halogen PAR16 lamp owners.
If the lamp submitted for an exchange is not a lamp included in this recall program, Philips will not send replacement product and will not be returning your lamp.
