How one clinically integrated network is managing care to succeed in VBC

Eastside Health Network (EHN), a joint venture Clinically Integrated Network (CIN) in Washington state, is working with Philips Wellcentive to take on progressively greater risk in its value-based care contracts, including full capitation.

The presenters will discuss the core tenets of EHN’s success and demonstrate how they’ve applied these to succeed in four different types of risk-based contracts. They’ll also discuss the importance of creating a shared culture with clinicians that aligns clinical and financial goals and celebrates practice wins.

Watch this webinar and learn how to:

  • Deliver trustworthy data aggregated from disparate sources in the form of a longitudinal record, making it easy for clinicians to take action
  • Focus on quality measures and set goals tailored to specific contracts
  • Proactively manage care, close care gaps and address patient leakage
  • Activate and engage patients and providers to collaboratively improve health
