This course will extensively cover the left ventricular outflow tract view, the right ventricular outflow tract view, the three-vessel view and the three-vessel trachea view. The anatomical relationships that must be imaged in each view will be discussed. These views are consistent with the 2018 AIUM updated OB ultrasound guidelines.  A checklist for normalcy of these views will be described.

Course Objectives:
Upon completion of this course, attendees should be able to:
  • State the checklist elements necessary to show normalcy of the outflow tract views
  • Discuss the normal anatomy of the fetal outflow tracts
  • Describe the outflow tract sweep that allows visualization of the required views

Course Description:
This course will extensively cover the four-chamber view and the anatomical relationship elements that need to be evaluated during the second trimester anatomy scan. A four-chamber checklist will be described that includes the evaluation of: situs, cardiac axis, cardiac position, cardiothoracic ratio, ventricular and atrial chamber sizes, moderator band detection in the right ventricle, atrioventricular valve arrangement at the crux of the heart, the interventricular and interatrial septum, and pulmonary vein detection.

Course Objectives:
Upon completion of this course, attendees should be able to:

  • List the main elements that should be evaluated during the examination of the 4-chamber view
  • Discuss the normal anatomy of the fetal heart
  • Describe normal blood flow through the fetal heart
  • Describe the relationship between chamber size and blood volume in the fetal heart

Course Description:
This lecture will cover basic fetal cardiac embryology in order to better understand how the heart structures form and how we must prove normalcy of the segments of the heart attaching to each other during fetal heart screening. Fetal circulation will be discussed along with the 3 fetal circulatory shunts that are unique to the fetus. Detailed fetal heart chamber-by-chamber anatomy will be discussed to aid the sonographer in the fetal heart evaluation. This lecture is an important, however optional, prerequisite for the 4 chamber and Outflow Tract lectures.

Course Objectives:
  • Discuss fetal heart embryologic “looping” and how this is necessary for the final location of the ventricles.
  • List the 3 shunts in fetal circulation
  • Describe cardiac anatomy including the chambers, valves, papillary muscles and chordae tendinae
  • Discuss the “spiraling” of the truncus arteriosus during embryologic development to form the great vessels
