    Extubation failure carries significant morbidity and mortality, and current evidence-based guidelines recommend NIV for select patients following extubation. This session examines the evidence behind those recommendations, explores patient selection criteria, and offers practical guidance on interface selection and the role of high-flow nasal oxygen as a complementary or alternative strategy. By the end of this session, participants will be able to:

    At a glance

    • Differentiate between prophylactic and rescue NIV use post-extubation, and identify which patient populations are most likely to benefit from each approach.
    • Compare the evidence for NIV versus high-flow nasal oxygen post-extubation, and describe how these therapies can be used independently or in combination based on patient need and clinical context.
    • Select an appropriate NIV interface and settings, recognize signs of treatment failure, and apply a structured approach to monitoring that reduces delays to reintubation when indicated.
