Medicare Hubs Connect Patients with Social Services
The Centers for Medicare & Medicaid Services (CMS) has chosen 32 community organizations throughout the US to help local Medicare and Medicaid patients gain easier access to social services that can help improve their health.
The organizations, which include providers and universities, will serve as hubs of information and assistance that will help identify the clinical needs of patients for housing, transportation, and food, which can significantly affect health outcomes and utilization of health resources. The organizations will also identify providers and other groups that can help fill such needs.
The five-year demonstration program, which begins in May, 2017, is called the Accountable Health Communities Model.
