Description: This recall involves Endura 12-watt and Ambient 12.5-watt LED dimmable light bulbs. The bulbs are orange in color and have “MADE IN CHINA,” “Fabrique in Chine” followed by a slanted “S,” and the model number 9290001829 printed on the gray plastic band on the neck of the bulbs. The date codes, 2L for the Endura bulbs and 2K or 2L for the Ambient bulbs, are printed on the metal screw base. The bulbs give off a white light and are used indoors to replace incandescent bulbs.
Remedy: Consumers should immediately stop using the recalled LED bulbs, unplug the fixture and remove the bulb and contact Philips at (800) 634-0138 between 9 a.m. and 5 p.m. ET Monday through Friday or email the firm at email@example.com for free replacement bulbs.
Sold at: Home Depot, grocery and home center stores nationwide, online retailers, including Amazon.com, and through electrical distributors between October 2012 and May 2013 for between $15 and $30.

Philips has identified a shock hazard involving the power resistor lead wire. This potentially lead to an improper fit of the lead wire in the lamp housing. In a small percentage of lamps the lead wire can come into physical contact with the lamp housing, which in turn can cause the housing to electrify. The following Products are affected by this recall:
Philips is committed to the well-being of its consumers and has decided to voluntarily recall the affected EnduraLED and AmbientLED lamps and replace them.
Philips is offering to exchange affected lamps and provide consumers with a free replacement lamp.
Yes. If you own one or more Philips EnduraLED and/or AmbientLED lamps from the affected batch, you will receive free replacement product.
Our advice to owners is to check whether their Philips EnduraLED and/or AmbientLED lamp is part of the affected batch. If your LED lamp is part of the affected batch, please immediately discontinue using the product and call (800) 634-0138 or send an email to firstname.lastname@example.org to register for a free replacement.

The following Products are affected by this recall:
No, the risk is limited to a batch of EnduraLED and AmbientLED lamps produced at one manufacturing location with the specific model number, date codes, and factory code as indicated above. All other EnduraLED and AmbientLED lamps are not at risk and completely safe to use.
The UPC number can be found on the Bar Code on the lamp’s package. The Bar Code, without the first digit, contains the UPC code that coincides with the list of products that is included in the recall.
For blister box product, the UPC number can be found on the bottom of the box (Figure1).
The Model Number can be found on the lamp itself in the grey area under Made in China.
The date code can be found on the plastic housing near the metal screw base.

The ordering code can be found on the back panel of the package below the model number.
Once you have registered your affected lamp and Philips has received and confirmed it was from an affected batch, you will be sent a replacement lamp. You can expect to receive your replacement LED lamp approximately four weeks after you make contact with Philips Customer Care either via phone @ (800) 634-0138, via E-mail @ email@example.com or by visiting the company’s website at www.philips.com/recall. We appreciate your cooperation and sincerely regret any inconvenience caused.
The CPSC is aware of the problem and we are working jointly with them to provide information to Philips EnduraLED and/or AmbientLED lamp owners.
If the lamp submitted for an exchange is not a lamp included in this recall program, Philips will not send replacement product and will not be returning your lamp.
